Nakon što je tri tjedna proveo na odjelu za intenzivnu skrb na kojemu se liječio od covida-19, liječnici su američkome kazališnom i televizijskom glumcu Nicku Coroderu (41) morali amputirati desnu nogu. Njegovu je cirkulaciju zaustavio ugrušak - još jedna opasna komplikacija bolesti koja je zahvatila cijeli svijet. 

Ugrušci su iz raznih razloga česta komplikacija među bolesnicima na intenzivnom liječenju, no njihova je učestalost veća među oboljelima od covida nego što je očekivano. 

"Na odjelu bolnice u kojoj radim imam četrdesetogodišnjake s ugrušcima u prstima, a nema nikakvog razloga za to osim virusa", rekla je Shari Broshahan, liječnica na odjelu intenzivne njege njujorške bolnice Langone.

Jedan od njih upravo ima velikih problema s dotokom krvi u obje noge i u obje ruke i vjerojatno ćemo morati obaviti amputaciju, rekla je. 

Krvni ugrušci nisu opasni samo za udove, nego mogu dospjeti i u pluća, srce ili mozak te rezultirati  smrtonosnom plućnom embolijom te srčanim i moždanim udarom.

U nedavnoj studiji nizozemskih znanstvenika otkriveno je da je 31 od 184 pacijenta uključena u istraživanje imao komplikacije s ugrušcima, što je "iznimno visok postotak". 

Na sreću, potreba za amputacijama ipak je puno rjeđa. 

Zašto se to događa? 

Behnood Bikdeli, liječnik u njujorškoj Prezbiterijanskoj bolnici, okupio je međunarodni tim stručnjaka da bi proučili problem. Njihova otkrića objavljena su u časopisu Journal of The American College of Cardiology.

Zaključili su da je rizik od pojave ugrušaka toliko visok da bi pacijenti oboljeli od covida -19 "trebali preventivno dobivati lijekove za razrjeđivanje krvi". 

Nekoliko je mogućih razloga koji ih uzrokuju. Pacijenti s težim oblikom covida-19 često otprije pate od neke bolesti, poput bolesti srca ili pluća. One su same po sebi povezane s većim rizikom od razvoja tromboze. 

I boravak na odjelu intenzivne skrbi povećava rizik jer pacijenti ondje dulje vrijeme nepomično leže, što pogoduje razvoju ugrušaka. 

Covid je povezan i s neuobičajenom reakcijom imunološkog sustava koja se naziva "citokinskom olujom", a neka istraživanju upućuju na to da je i taj fenomen povezan s višom stopom stvaranja trombova. 

Moguće je i da na zgrušavanje utječe nešto povezano sa samim koronavirusom, što su liječnici zamijetili ranije kod nekih drugih virusnih oboljenja. 

Stručni rad objavljen prošli tjedan u Lancetu pokazao je da virus može nastaniti epitel, tkivo koje oblaže organe i krvne žile, što u teoriji može utjecati na proces zgrušavanja. 

Mikrougrušci

Premda antikoagulansi, poput heparina kod nekih pacijenata djeluju vrlo učinkovito, kod drugih nisu djelotvorni jer su ugrušci premaleni, pojasnila je Brosnahan. "Mikrougrušaka je puno i ne znamo gdje se točno nalaze." Obdukcije su pokazale da su pluća nekih pacijenata sadržavala stotine mikrougrušaka.

No pojava nove zagonetke riješila je jednu stariju. Cecilia Mirant-Borde, liječnica na odjelu za intenzivnu skrb bolnice za vojne veterane na Manhattanu kaže da pluća puna mikrougrušaka objašnjavaju zbog čega respiratori nisu bili od veće pomoći kod jednog dijela pacijenata. 

Na početku pandemije liječnici su te pacijente liječili prema protokolu za akutni respiratorni distres sindrom, poznat i kao "mokra pluća". 

Razlog neučinkovitosti respiratora u nekim slučajevima "nisu bila pluća puna vode, nego mikrougrušci koji su blokirali cirkulaciju" pa je krv napuštala pluća s manje kisika nego što je to uobičajeno.  

Prošlo je pet mjeseci otkako se virus pojavio u Wuhanu, a liječnici o njemu svakoga dana doznaju više.

"Premda smo iznenađeni otkrićem, zapravo ne bismo trebali biti toliko iznenađeni. Virusi su nepredvidljivi", rekla je Brosnahan.

Nakon što je tri tjedna proveo na odjelu za intenzivnu skrb na kojemu se liječio od covida-19, liječnici su američkome kazališnom i televizijskom glumcu Nicku Coroderu (41) morali amputirati desnu nogu. Njegovu je cirkulaciju zaustavio ugrušak - još jedna opasna komplikacija bolesti koja je zahvatila cijeli svijet. 

Ugrušci su iz raznih razloga česta komplikacija među bolesnicima na intenzivnom liječenju, no njihova je učestalost veća među oboljelima od covida nego što je očekivano. 

"Na odjelu bolnice u kojoj radim imam četrdesetogodišnjake s ugrušcima u prstima, a nema nikakvog razloga za to osim virusa", rekla je Shari Broshahan, liječnica na odjelu intenzivne njege njujorške bolnice Langone.

Jedan od njih upravo ima velikih problema s dotokom krvi u obje noge i u obje ruke i vjerojatno ćemo morati obaviti amputaciju, rekla je. 

Krvni ugrušci nisu opasni samo za udove, nego mogu dospjeti i u pluća, srce ili mozak te rezultirati  smrtonosnom plućnom embolijom te srčanim i moždanim udarom.

U nedavnoj studiji nizozemskih znanstvenika otkriveno je da je 31 od 184 pacijenta uključena u istraživanje imao komplikacije s ugrušcima, što je "iznimno visok postotak". 

Na sreću, potreba za amputacijama ipak je puno rjeđa. 

Zašto se to događa? 

Behnood Bikdeli, liječnik u njujorškoj Prezbiterijanskoj bolnici, okupio je međunarodni tim stručnjaka da bi proučili problem. Njihova otkrića objavljena su u časopisu Journal of The American College of Cardiology.

Zaključili su da je rizik od pojave ugrušaka toliko visok da bi pacijenti oboljeli od covida -19 "trebali preventivno dobivati lijekove za razrjeđivanje krvi". 

Nekoliko je mogućih razloga koji ih uzrokuju. Pacijenti s težim oblikom covida-19 često otprije pate od neke bolesti, poput bolesti srca ili pluća. One su same po sebi povezane s većim rizikom od razvoja tromboze. 

I boravak na odjelu intenzivne skrbi povećava rizik jer pacijenti ondje dulje vrijeme nepomično leže, što pogoduje razvoju ugrušaka. 

Covid je povezan i s neuobičajenom reakcijom imunološkog sustava koja se naziva "citokinskom olujom", a neka istraživanju upućuju na to da je i taj fenomen povezan s višom stopom stvaranja trombova. 

Moguće je i da na zgrušavanje utječe nešto povezano sa samim koronavirusom, što su liječnici zamijetili ranije kod nekih drugih virusnih oboljenja. 

Stručni rad objavljen prošli tjedan u Lancetu pokazao je da virus može nastaniti epitel, tkivo koje oblaže organe i krvne žile, što u teoriji može utjecati na proces zgrušavanja. 

Mikrougrušci

Premda antikoagulansi, poput heparina kod nekih pacijenata djeluju vrlo učinkovito, kod drugih nisu djelotvorni jer su ugrušci premaleni, pojasnila je Brosnahan. "Mikrougrušaka je puno i ne znamo gdje se točno nalaze." Obdukcije su pokazale da su pluća nekih pacijenata sadržavala stotine mikrougrušaka.

No pojava nove zagonetke riješila je jednu stariju. Cecilia Mirant-Borde, liječnica na odjelu za intenzivnu skrb bolnice za vojne veterane na Manhattanu kaže da pluća puna mikrougrušaka objašnjavaju zbog čega respiratori nisu bili od veće pomoći kod jednog dijela pacijenata. 

Na početku pandemije liječnici su te pacijente liječili prema protokolu za akutni respiratorni distres sindrom, poznat i kao "mokra pluća". 

Razlog neučinkovitosti respiratora u nekim slučajevima "nisu bila pluća puna vode, nego mikrougrušci koji su blokirali cirkulaciju" pa je krv napuštala pluća s manje kisika nego što je to uobičajeno.  

Prošlo je pet mjeseci otkako se virus pojavio u Wuhanu, a liječnici o njemu svakoga dana doznaju više.

"Premda smo iznenađeni otkrićem, zapravo ne bismo trebali biti toliko iznenađeni. Virusi su nepredvidljivi", rekla je Brosnahan.

Novo smrtonosno iznenađenje covida-19: liječnici začuđeni krvnim ugrušcima kod pacijenata, oboljelima prijete amputacije udova, moždani i srčani udari

Nakon što je tri tjedna proveo na odjelu za intenzivnu skrb na kojemu se liječio od covida-19, liječnici su američkome kazališnom i televizijskom glumcu Nicku Coroderu (41) morali amputirati desnu nogu. Njegovu je cirkulaciju zaustavio ugrušak - još jedna opasna komplikacija bolesti koja je zahvatila cijeli svijet. 

Ugrušci su iz raznih razloga česta komplikacija među bolesnicima na intenzivnom liječenju, no njihova je učestalost veća među oboljelima od covida nego što je očekivano. 

"Na odjelu bolnice u kojoj radim imam četrdesetogodišnjake s ugrušcima u prstima, a nema nikakvog razloga za to osim virusa", rekla je Shari Broshahan, liječnica na odjelu intenzivne njege njujorške bolnice Langone.

Jedan od njih upravo ima velikih problema s dotokom krvi u obje noge...
