Liječnici od Italije do Sjedinjenih Država nekoliko zadnjih tjedana raspravljaju o tome kada je pravi trenutak da teško oboljele pacijente od covida-19 stave na respirator.
Riječ je o jednom od aktualnih pitanja koje sada nije ništa manje važno od rasprave o učinkovitosti hidroksiklorokina, rekao je američki liječnik za agenciju AFP.
U jeku pandemije studije su nedostatne i nemoguće je znati bi li osobe koje su stavljene na respirator svejedno umrle zbog težine njihova stanja.
Međutim, brojni liječnici konstatirali su da se stanje dijela oboljelih od covida-19 rapidno pogoršalo nakon intubacije i stavljanja na respirator, zbog čega već nekoliko tjedana američke bolnice odgađaju taj postupak iako je, u strahu od nestašice, američka vlada nedavno naručila više od 130.000 tih strojeva za disanje.
Prva upozorenja stigla su iz Italije gdje je umrla velika većina pacijenata koja je završila na respiratoru.
Statistike su jednako neumoljive i u Velikoj Britaniji te u New Yorku gdje je 80 posto intubiranih pacijenata umrlo, prema riječima guvernera te države koji navodi da je smrt često nastupila tjedan ili dva nakon tog postupka, a to je dug period koji oslabi tijelo jer su gotovo svi pacijenti stavljeni u umjetnu komu i atrofiraju im mišići.
Prema pacijentima koji su ostali bez daha na početku pandemije postupalo se prema dobro uhodanom protokolu za akutni respiratorni distresni sindrom (ARDS).
To stanje koje sprječava pluća da uzmu dovoljno kisika za opskrbu organa, može biti izazvano infekcijom ili fizičkom ozljedom. ARDS je vrlo teško stanje i smrtnost mu je oko 40 posto.
Uhodana je praksa da se takvi pacijenti relativno brzo intubiraju.
Takav se protokol primjenjivao i za oboljele od koronavirusa sve dok liječnici nisu ustanovili da sindrom covida-19 nije posve identičan "uobičajenom" respiratnornom distresu, barem ne za dio oboljelih.
Pluća nisu na jednak način oštećena, odnosno manje su kruta.
Liječnik Luciano Gattinoni i njegovi kolege iz Milana opisali su krajem ožujka kako su prilagodili postupak.
"Sve što možemo učiniti ventiliranjem tih pacijenata je 'kupiti vrijeme' s minimalnom dodatnom štetom", napisao je Gattinoni u časopisu American Thoracic Society.
"Moramo biti strpljivi", kazao je. "Učimo u hodu"
Kevin Wilson, profesor medicine na bostonskom sveučilištu i član američkog torokalnog društva, slaže se da je potreban oprez.
"Većina zdravstvene zajednice postala je nervozna izvješćima da pacijentima ne ide tako dobro na respiratorima i pokušavaju odgoditi intubaciju", rekao je.
"Odgađamo koliko možemo, ali ne do točke hitnosti", dodao je.
Liječnici su ustanovili da ima pacijenata kojima je postotak kisika u krvi jako opao, što bi u normalnim uvjetima zahtijevalo intubaciju, ali koji zapravo nisu u tako lošem stanju.
Umjesto da brzo intubiraju, liječnici se koriste drugim postupcima davanja kisika: nazalnim kanilama (male cjevčice koje se stavljaju u nosnice), konvencionalnim ili sofisticiranijim maskama s kisikom ili polijeganjem oboljelog na trbuh jer to pomaže plućima.
"Učimo u hodu", objašnjava Wilson.
To se događa i u New Yorku gdje je virus pokosio više od 10.000 ljudskih života.
"Pokušavamo malo dulje pričekati, ako je moguće, pa tek onda intubirati", rekao je za AFP Daniel Griffin, epidemiolog za ProHealth Care, mrežu više od tisuću liječnika koji rade u dvadesetak bolnica u New Yorku.
"Ako se ocijeni da pacijenti još mogu izdržati, daju im se manje količine kisika i neki od njih tako se uspijevaju oporaviti bez respiratora", kazao je.
Ako pak pacijenti ipak trebaju respirator, Griffin objašnjava da se mogu različito podesiti tako da opskrbljuju kisikom pod manjim tlakom.
Medicinska društva upravo rade na smjernicama za najbolju upotrebu, ali nitko još nema konačan odgovor.

Liječnici od Italije do Sjedinjenih Država nekoliko zadnjih tjedana raspravljaju o tome kada je pravi trenutak da teško oboljele pacijente od covida-19 stave na respirator.
Riječ je o jednom od aktualnih pitanja koje sada nije ništa manje važno od rasprave o učinkovitosti hidroksiklorokina, rekao je američki liječnik za agenciju AFP.
U jeku pandemije studije su nedostatne i nemoguće je znati bi li osobe koje su stavljene na respirator svejedno umrle zbog težine njihova stanja.
Međutim, brojni liječnici konstatirali su da se stanje dijela oboljelih od covida-19 rapidno pogoršalo nakon intubacije i stavljanja na respirator, zbog čega već nekoliko tjedana američke bolnice odgađaju taj postupak iako je, u strahu od nestašice, američka vlada nedavno naručila više od 130.000 tih strojeva za disanje.
Prva upozorenja stigla su iz Italije gdje je umrla velika većina pacijenata koja je završila na respiratoru.
Statistike su jednako neumoljive i u Velikoj Britaniji te u New Yorku gdje je 80 posto intubiranih pacijenata umrlo, prema riječima guvernera te države koji navodi da je smrt često nastupila tjedan ili dva nakon tog postupka, a to je dug period koji oslabi tijelo jer su gotovo svi pacijenti stavljeni u umjetnu komu i atrofiraju im mišići.
Prema pacijentima koji su ostali bez daha na početku pandemije postupalo se prema dobro uhodanom protokolu za akutni respiratorni distresni sindrom (ARDS).
To stanje koje sprječava pluća da uzmu dovoljno kisika za opskrbu organa, može biti izazvano infekcijom ili fizičkom ozljedom. ARDS je vrlo teško stanje i smrtnost mu je oko 40 posto.
Uhodana je praksa da se takvi pacijenti relativno brzo intubiraju.
Takav se protokol primjenjivao i za oboljele od koronavirusa sve dok liječnici nisu ustanovili da sindrom covida-19 nije posve identičan "uobičajenom" respiratnornom distresu, barem ne za dio oboljelih.
Pluća nisu na jednak način oštećena, odnosno manje su kruta.
Liječnik Luciano Gattinoni i njegovi kolege iz Milana opisali su krajem ožujka kako su prilagodili postupak.
"Sve što možemo učiniti ventiliranjem tih pacijenata je 'kupiti vrijeme' s minimalnom dodatnom štetom", napisao je Gattinoni u časopisu American Thoracic Society.
"Moramo biti strpljivi", kazao je. "Učimo u hodu"
Kevin Wilson, profesor medicine na bostonskom sveučilištu i član američkog torokalnog društva, slaže se da je potreban oprez.
"Većina zdravstvene zajednice postala je nervozna izvješćima da pacijentima ne ide tako dobro na respiratorima i pokušavaju odgoditi intubaciju", rekao je.
"Odgađamo koliko možemo, ali ne do točke hitnosti", dodao je.
Liječnici su ustanovili da ima pacijenata kojima je postotak kisika u krvi jako opao, što bi u normalnim uvjetima zahtijevalo intubaciju, ali koji zapravo nisu u tako lošem stanju.
Umjesto da brzo intubiraju, liječnici se koriste drugim postupcima davanja kisika: nazalnim kanilama (male cjevčice koje se stavljaju u nosnice), konvencionalnim ili sofisticiranijim maskama s kisikom ili polijeganjem oboljelog na trbuh jer to pomaže plućima.
"Učimo u hodu", objašnjava Wilson.
To se događa i u New Yorku gdje je virus pokosio više od 10.000 ljudskih života.
"Pokušavamo malo dulje pričekati, ako je moguće, pa tek onda intubirati", rekao je za AFP Daniel Griffin, epidemiolog za ProHealth Care, mrežu više od tisuću liječnika koji rade u dvadesetak bolnica u New Yorku.
"Ako se ocijeni da pacijenti još mogu izdržati, daju im se manje količine kisika i neki od njih tako se uspijevaju oporaviti bez respiratora", kazao je.
