Većina njemačkih klinika završit će godinu u minusu. Razlog: manje je pacijenata jer su kapaciteti oslobođeni za oboljele od Covida – a njih nije bilo baš toliko. Kritičari pak kažu da pacijenata inače ima previše, piše DW.

Njemačka ima četiri puta više kreveta na odjelima intenzivne njege od Velike Britanije, šest puta više od Švedske i dvostruko više od Francuske. Ipak, u Njemačkoj se boje prepunih jedinica intenzivne njege, pa čak i mogućnosti da će se morati uvesti trijaža, odnosno da će liječnici morati odlučivati koga ima smisla liječiti, a koga ne.

Za takvu situaciju postoji više razloga, kaže liječnik Reinhard Buse, profesor na Tehničkom sveučilištu Berlin i član Stručnog savjetodavnog odbora njemačkog Saveznog ministarstva zdravlja.

- Jedan je to što, u usporedbi s drugim zemljama, u Njemačkoj više pacijenata oboljelih od korone završi na odjelima intenzivne njege. Oko tri puta više nego primjerice u Nizozemskoj ili Danskoj, i čak četiri puta više nego u Velikoj Britaniji - rekao je doktor Buse za DW.

To se događa zato što je broj zaposlenih na odjelima intenzivne njege u Njemačkoj veći nego na drugim odjelima, objašnjava Buse.

- Danska to radi drugačije. Za svaki krevet na normalnom odjelu ona angažira gotovo isti broj medicinskog osoblja kao i na njemačkoj jedinici intenzivne njege. To ne znači da Danci imaju više medicinskog osoblja, već samo da ima manje nepotrebnih kreveta - napominje liječnik.

Bolnice u gubicima

Pritom čak 60 posto bolnica u Njemačkoj računa da će biti u financijskom minusu za 2021. godinu, pokazuje tzv. "Bolnički barometar" koji redovito objavljuje Njemačko udruženje bolnica. Kako se navodi, time se u odnosu na prošlu godinu udvostručuje broj klinika koje su u gubitcima.

Svaka druga bolnica ima manje pacijenata nego prošle godine. Razlog za to je otkazivanje operacija i kompliciranih zahvata kako bi se čuvali kapaciteti za Covid-odjele. Mnogi pacijenti odgađaju odlazak na terapije ili operacije iz straha od zaraze.

Kako navode u Udruženju bolnica, stanje je najlošije još od 2000. godine. Svega jedanaest posto bolnica trenutno ocjenjuje financijsku situaciju kao dobru. Za iduću godinu tek 22 posto očekuje poboljšanje.

"Zlatno doba" za bolnice?

- Njemačkim bolnicama je u interesu da budu pune, a to važi i za jedinice intenzivne njege - kaže Buse. On smatra da su posljednjih godinu i pol dana za bolnice "bile zlatne", jer su mnoge od njih zaradile više novca od paušalnih naknada i dodatnih kreveta intenzivne njege njego što bi to bilo moguće u normalnom radu.

- Riječ je o 14,5 milijardi eura za godinu i pol koji su se samo od poreznih prihoda slili u bolnice. A tu su i redovna sredstva iz obaveznog zdravstvenog osiguranja - naglašava doktor.

On ukazuje na to da polovica pacijenata koji se liječe u bolnicama u Njemačkoj dolazi tamo bez prijema, najčešće iz hitne pomoći.

 - Vjerojatnost da će bolnica zadržati takvog pacijenta na liječenju je skoro 50 posto, što je u usporedbi s drugim zemljama nevjerojatno mnogo. Obično je taj postotak upola manji, oko 25 posto - ističe Buse.

Osim toga, dodaje Buse, na njemačkim odjelima intenzivne njege ima teško bolesnih ljudi koji imaju jako male šanse za preživljavanje, čak i uz upotrebu najsuvremenije opreme.

- Prvo što bi trebalo učiniti nakon pandemije je utvrditi tko se sve liječi na odjelima intenzivne njege. Moramo se zapitati što mi tamo radimo - zaključuje doktor Buse. 

Manjak osoblja ili višak pacijenata?

Njemačko udruženje bolnica posebno ukazuje na problem manjka osoblja. Četiri petine klinika navode da imaju poteškoća s pronalaskom medicinskih sestara i tehničara za obične odjele i odjele intenzivne njege. Diljem Njemačke ima 22.300 zauzetih radnih mjesta za njegovatelje.

Broj nepopunjenih mjesta se utrostručio u posljednjih pet godina.

- Manjak osoblja je najhitniji problem zdravstvene politike - kaže predsjednik udruge Gerald Gas.

Reinhard Buse o tome ima drugačije mišljenje.

- Mi nemamo premalo osoblja, već previše pacijenata. U prosjeku je prije korone u bolnicama u Njemačkoj svakog dana bilo po 380 000 pacijenata, a trenutno ih je samo oko 325 000. Da smo imali strukturu kao u Danskoj, prije korone bi ih bilo 160 000, odnosno manje od pola. To znači da bi u Njemačkoj bilo duplo više medicinskog osoblja po pacijentu - kaže on.

- Ako pitate upravitelje bolnica, oni će vam reći: treba nam toliko bolnica, jer imamo toliko pacijenata. Ja mislim drugačije: imamo toliko pacijenata, jer imamo toliko bolnica. To je kao pitanje što je starije, kokoš ili jaje -  zaključuje Reinhard Buse za DW.

Većina njemačkih klinika završit će godinu u minusu. Razlog: manje je pacijenata jer su kapaciteti oslobođeni za oboljele od Covida – a njih nije bilo baš toliko. Kritičari pak kažu da pacijenata inače ima previše, piše DW.

Njemačka ima četiri puta više kreveta na odjelima intenzivne njege od Velike Britanije, šest puta više od Švedske i dvostruko više od Francuske. Ipak, u Njemačkoj se boje prepunih jedinica intenzivne njege, pa čak i mogućnosti da će se morati uvesti trijaža, odnosno da će liječnici morati odlučivati koga ima smisla liječiti, a koga ne.

Za takvu situaciju postoji više razloga, kaže liječnik Reinhard Buse, profesor na Tehničkom sveučilištu Berlin i član Stručnog savjetodavnog odbora njemačkog Saveznog ministarstva zdravlja.

- Jedan je to što, u usporedbi s drugim zemljama, u Njemačkoj više pacijenata oboljelih od korone završi na odjelima intenzivne njege. Oko tri puta više nego primjerice u Nizozemskoj ili Danskoj, i čak četiri puta više nego u Velikoj Britaniji - rekao je doktor Buse za DW.

To se događa zato što je broj zaposlenih na odjelima intenzivne njege u Njemačkoj veći nego na drugim odjelima, objašnjava Buse.

- Danska to radi drugačije. Za svaki krevet na normalnom odjelu ona angažira gotovo isti broj medicinskog osoblja kao i na njemačkoj jedinici intenzivne njege. To ne znači da Danci imaju više medicinskog osoblja, već samo da ima manje nepotrebnih kreveta - napominje liječnik.

Bolnice u gubicima

Pritom čak 60 posto bolnica u Njemačkoj računa da će biti u financijskom minusu za 2021. godinu, pokazuje tzv. "Bolnički barometar" koji redovito objavljuje Njemačko udruženje bolnica. Kako se navodi, time se u odnosu na prošlu godinu udvostručuje broj klinika koje su u gubitcima.

Svaka druga bolnica ima manje pacijenata nego prošle godine. Razlog za to je otkazivanje operacija i kompliciranih zahvata kako bi se čuvali kapaciteti za Covid-odjele. Mnogi pacijenti odgađaju odlazak na terapije ili operacije iz straha od zaraze.

Kako navode u Udruženju bolnica, stanje je najlošije još od 2000. godine. Svega jedanaest posto bolnica trenutno ocjenjuje financijsku situaciju kao dobru. Za iduću godinu tek 22 posto očekuje poboljšanje.

"Zlatno doba" za bolnice?

- Njemačkim bolnicama je u interesu da budu pune, a to važi i za jedinice intenzivne njege - kaže Buse. On smatra da su posljednjih godinu i pol dana za bolnice "bile zlatne", jer su mnoge od njih zaradile više novca od paušalnih naknada i dodatnih kreveta intenzivne njege njego što bi to bilo moguće u normalnom radu.

- Riječ je o 14,5 milijardi eura za godinu i pol koji su se samo od poreznih prihoda slili u bolnice. A tu su i redovna sredstva iz obaveznog zdravstvenog osiguranja - naglašava doktor.

On ukazuje na to da polovica pacijenata koji se liječe u bolnicama u Njemačkoj dolazi tamo bez prijema, najčešće iz hitne pomoći.

 - Vjerojatnost da će bolnica zadržati takvog pacijenta na liječenju je skoro 50 posto, što je u usporedbi s drugim zemljama nevjerojatno mnogo. Obično je taj postotak upola manji, oko 25 posto - ističe Buse.

Osim toga, dodaje Buse, na njemačkim odjelima intenzivne njege ima teško bolesnih ljudi koji imaju jako male šanse za preživljavanje, čak i uz upotrebu najsuvremenije opreme.

- Prvo što bi trebalo učiniti nakon pandemije je utvrditi tko se sve liječi na odjelima intenzivne njege. Moramo se zapitati što mi tamo radimo - zaključuje doktor Buse. 

Manjak osoblja ili višak pacijenata?

Njemačko udruženje bolnica posebno ukazuje na problem manjka osoblja. Četiri petine klinika navode da imaju poteškoća s pronalaskom medicinskih sestara i tehničara za obične odjele i odjele intenzivne njege. Diljem Njemačke ima 22.300 zauzetih radnih mjesta za njegovatelje.

Broj nepopunjenih mjesta se utrostručio u posljednjih pet godina.

- Manjak osoblja je najhitniji problem zdravstvene politike - kaže predsjednik udruge Gerald Gas.

Reinhard Buse o tome ima drugačije mišljenje.

- Mi nemamo premalo osoblja, već previše pacijenata. U prosjeku je prije korone u bolnicama u Njemačkoj svakog dana bilo po 380 000 pacijenata, a trenutno ih je samo oko 325 000. Da smo imali strukturu kao u Danskoj, prije korone bi ih bilo 160 000, odnosno manje od pola. To znači da bi u Njemačkoj bilo duplo više medicinskog osoblja po pacijentu - kaže on.

- Ako pitate upravitelje bolnica, oni će vam reći: treba nam toliko bolnica, jer imamo toliko pacijenata. Ja mislim drugačije: imamo toliko pacijenata, jer imamo toliko bolnica. To je kao pitanje što je starije, kokoš ili jaje -  zaključuje Reinhard Buse za DW.
