Prof.prim. dr.sc. Branko Kolarić iz Nastavnog zavoda za javno zdravstvo "Dr.Andrija Štampar" je u suradnji s Hrvatskim zavodom za javno zdravstvo i Veterinarskim fakultetom i Gradskim uredom za socijalnu zaštitu i osobe s invaliditetom proveo serološko istraživanje u jednom domu za starije u Zagrebu, javlja N1. 

Radi se o znantsvenom istraživanju koje je dobilo dozvole etičkog povjerenstva, a u svrhu kojeg su korisnici doma darovali uzorke krvi šest mjeseci nakon cijepljenja drugom dozom. Korištena je standardna metoda određivanje protutijela, ali se radio i zahtjevniji test detekcije neutralizacijskih protutijela. Radi se testu koji se provodi na stanicama bubrega zelenog afričkog majmuna u kojima se nalaze živi koronavirusi i njime možemo detektirati da li protutijela koja detektiramo zaista ubijaju virus.

Preliminarni rezultati govore da oko 30% ispitanika šest mjeseci nakon druge doze više nije imalo zaštitnu razinu protutijela što pokazuje da bi ta, najosjetljivija skupina uskoro mogla imati potrebu za nadocjepljivanjem. 

- Od početka epidemije bavim se populacijom starijih osoba, a posebice onima u domovima za starije. To je skupina koja je najugroženija: u dubokoj su starosti (prosjek godina 83), imaju mnoge druge bolesti, a i nalaze se u zatvorenom kolektivu gdje se epidemija efikasnije širi. Ako se sjećate, prvo cijepljenje bilo je u domu za starije 27.prosinca prošle godine, a korisnici domova bili su najviša kategorija prioriteta.

Žao mi je da već tada nismo krenuli s ovakvim istraživanjem, no sada su me potakle rasprave kolika je dužina zaštite od cjepiva. Bilo je predviđanja od toga da će se razviti doživotni imunitet do onih da će zaštita trajati dva do tri mjeseca- kazao je dr.sc. Branko Kolarić za Večernji list, ističući kako se ovdje radi o preliminarnim rezultatima koji govore da nakon šest mjeseci oko 30% korisnika domova za starije više nemaju zaštitu od virusa, a još 20% ima nizak titar i vjerojatno će kroz kraće vrijeme ostati bez protutijela te napominje da govori samo o populaciji korisnika domova za starije dok rezultati za mlađe osobe ukazuju na duže trajanje zaštite.

Upitan je i znači li to da je cjepivo neučinkovito. 

- Ne. Cjepivo je efikasno! To smo vidjeli po razlici pobolijevanja i umiranja u drugom i trećem valu. Nakon cijepljenja 80% korisnika broj umrlih u trećem valu bio je 20 puta manji. No starije osobe imaju slabiji, sporiji i kratkotrajniji imuni odgovor. To se naziva imunosenescencija. Još uvijek je vrlo malo rezultata ovakvih istraživanja te će i naše istraživanje doprinijeti odluci o uvođenju treće doze cjepiva za populaciju korisnika domova.

Također smo radili i test neutralizacijskih protutijela. Taj test se provodi na Vero E6 stanicama, stanicama epitela bubrega afričkog zelenog majmuna, na kojima se u posebno zaštićenim uvjetima uzgaja živi koronavirus potreban za istraživanja. Ti rezultati su vjerodostojniji od rutinskog testiranja i zbog zahtjevnosti se ne provode u puno centara- kazao je.

- Procjenjujem da će za osobe u domovima za starije to biti relativno brzo, u pitanju su tjedni a ne mjeseci. Za mlađe se još neće donositi takve preporuke. Vode se i rasprave da li za treću dozu dati isto cjepivo kao u prve dvije. Korisnici domova za starije cijepljeni su mRNA cjepivo pa sam stava da bi u booster dozi mogli dobiti adenovirusno cjepivo kako bi se na malo različit način potaknuo imunološki odgovor- zaključio je prof.prim. dr.sc. Branko Kolarić te na kraju zahvalio puno timu profesorice Vilibić-Čavlek (HZJZ), prof. Jasmine Vraneš (NZJZ), prof Ljubi Barbiću (Veterinarski fakultet) te pročelnici Gradskog ureda za socijalnu zaštitu i osoba se s invaliditetom.

Radi se o znantsvenom istraživanju koje je dobilo dozvole etičkog povjerenstva, a u svrhu kojeg su korisnici doma darovali uzorke krvi šest mjeseci nakon cijepljenja drugom dozom. Korištena je standardna metoda određivanje protutijela, ali se radio i zahtjevniji test detekcije neutralizacijskih protutijela. Radi se testu koji se provodi na stanicama bubrega zelenog afričkog majmuna u kojima se nalaze živi koronavirusi i njime možemo detektirati da li protutijela koja detektiramo zaista ubijaju virus.

Preliminarni rezultati govore da oko 30% ispitanika šest mjeseci nakon druge doze više nije imalo zaštitnu razinu protutijela što pokazuje da bi ta, najosjetljivija skupina uskoro mogla imati potrebu za nadocjepljivanjem. 

- Od početka epidemije bavim se populacijom starijih osoba, a posebice onima u domovima za starije. To je skupina koja je najugroženija: u dubokoj su starosti (prosjek godina 83), imaju mnoge druge bolesti, a i nalaze se u zatvorenom kolektivu gdje se epidemija efikasnije širi. Ako se sjećate, prvo cijepljenje bilo je u domu za starije 27.prosinca prošle godine, a korisnici domova bili su najviša kategorija prioriteta.

Žao mi je da već tada nismo krenuli s ovakvim istraživanjem, no sada su me potakle rasprave kolika je dužina zaštite od cjepiva. Bilo je predviđanja od toga da će se razviti doživotni imunitet do onih da će zaštita trajati dva do tri mjeseca- kazao je dr.sc. Branko Kolarić za Večernji list, ističući kako se ovdje radi o preliminarnim rezultatima koji govore da nakon šest mjeseci oko 30% korisnika domova za starije više nemaju zaštitu od virusa, a još 20% ima nizak titar i vjerojatno će kroz kraće vrijeme ostati bez protutijela te napominje da govori samo o populaciji korisnika domova za starije dok rezultati za mlađe osobe ukazuju na duže trajanje zaštite.

Upitan je i znači li to da je cjepivo neučinkovito. 

- Ne. Cjepivo je efikasno! To smo vidjeli po razlici pobolijevanja i umiranja u drugom i trećem valu. Nakon cijepljenja 80% korisnika broj umrlih u trećem valu bio je 20 puta manji. No starije osobe imaju slabiji, sporiji i kratkotrajniji imuni odgovor. To se naziva imunosenescencija. Još uvijek je vrlo malo rezultata ovakvih istraživanja te će i naše istraživanje doprinijeti odluci o uvođenju treće doze cjepiva za populaciju korisnika domova.

Također smo radili i test neutralizacijskih protutijela. Taj test se provodi na Vero E6 stanicama, stanicama epitela bubrega afričkog zelenog majmuna, na kojima se u posebno zaštićenim uvjetima uzgaja živi koronavirus potreban za istraživanja. Ti rezultati su vjerodostojniji od rutinskog testiranja i zbog zahtjevnosti se ne provode u puno centara- kazao je.

- Procjenjujem da će za osobe u domovima za starije to biti relativno brzo, u pitanju su tjedni a ne mjeseci. Za mlađe se još neće donositi takve preporuke. Vode se i rasprave da li za treću dozu dati isto cjepivo kao u prve dvije. Korisnici domova za starije cijepljeni su mRNA cjepivo pa sam stava da bi u booster dozi mogli dobiti adenovirusno cjepivo kako bi se na malo različit način potaknuo imunološki odgovor- zaključio je prof.prim. dr.sc. Branko Kolarić te na kraju zahvalio puno timu profesorice Vilibić-Čavlek (HZJZ), prof. Jasmine Vraneš (NZJZ), prof Ljubi Barbiću (Veterinarski fakultet) te pročelnici Gradskog ureda za socijalnu zaštitu i osoba se s invaliditetom.
