Moderna, jedna od tvrtki koja radi na cjepivu protiv koronavirusa, objavila je najnovije rezultate ispitivanja faze I njihovog RNA cjepiva. Na prvi pogled vijest je uzbudljiva: cjepivo izgleda sigurno, a dobrovoljci su razvili antitijela.
No prije nego što svi počnemo planirati što ćemo raditi nakon što svi primimo cjepivo, važno je ove rezultate staviti u odgovarajući kontekst. Evo što biste trebali znati.
Ispitivanje faze I osmišljeno je kako bi se ispitala sigurnost, a ne učinkovitost. Naime, ispitivanje faze I prvenstveno ispituje koliko je cjepivo sigurno, a ne da li je učinkovito.
U ispitivanju faze I zdravi dobrovoljci dobivaju različite količine cjepiva i nadgledaju se zbog bilo kakvih nuspojava. U ovo ispitivanje uključeno je 45 zdravih dobrovoljaca koji su primili različite razine doze cjepiva.
Zasad se čini da je cjepivo sigurno, a FDA je odobrila prelazak na ispitivanja faze II, gdje će dobrovoljcima koji su u visokom riziku od virusa dati terapeutsku dozu.
Uz ispitivanje sigurnosti različitih doza, istraživači su također pratili dobrovoljce na znakove da je cjepivo potaknulo imunološki odgovor testiranjem kako bi utvrdili razvijaju li se antitijela protiv virusa.
Moderna je najavila da je od 45 osoba uključenih u ispitivanje osam moglo biti testirano, a njihova krv imala je očekivana neutralizirajuća antitijela na razinama "na ili iznad" onih kakvi su se razvili kod ljudi koji su se oporavili od COVID-19.
"Ovo su sjajne vijesti", kaže John Cooke, liječnik znanstvenik iz Houstonske metodističke bolnice, koji ne sudjeluje u istraživanju komentirao je: "Ali, još uvijek ne znamo hoće li stvaranje antitijela zaštititi ljude od bolesti."

To što se cjepivo pokazalo sigurnim za osam ljudi još ne znači da je sigurno za milijune ljudi. Tu je i pitanje koliko će ovo cjepivo biti učinkovito: Trenutno istraživači proučavaju stimulira li imunološki odgovor stvaranje antitijela slična onima pronađenim u oporavljenih pacijenata. Iako je ovo obećavajući rezultat, postoji nekoliko ograničenja.
Kako ističe Peter Hotez, istaknuti znanstvenik za cjepiva i stručnjak za tropske bolesti, posjedovanje antitijela protiv virusa nije nužno dovoljno.
Samo to što kod pacijenta možemo pronaći antitijela protiv virusa ne mora nužno značiti da su to bila antitijela koja su bila učinkovita u borbi protiv virusa, tako da još treba obaviti puno posla kako bi se utvrdilo koliko je ovo cjepivo zaista sigurno i učinkovito. Također je problematično što je ove rezultate objavila sama Moderna, umjesto u recenziranoj studiji.
Tu je i mogući sukob interesa. Naime, jedan od direktora Moderne, Moncef Slaoui, istupio je iz upravnog odbora i postao glavni znanstvenik za inicijativu Bijele kuće za ubrzavanje razvoja cjepiva. Slaoui trenutno ima više od 10 milijuna dolara dionica u Moderni. Otkako su ove vijesti objavljene, vrijednost njegovih dionica skočila je za oko 25 posto.
Činjenica da preliminarni dokazi pokazuju da ovo cjepivo izgleda sigurno i da može stimulirati imunološki sustav dobar je znak. Također je vrijedno napomenuti da je FDA odobrila ovom kandidatu za cjepivo da započne ispitivanja faze II, a ispitivanja faze III procjenjuju se da će započeti u srpnju, ukoliko se ispitivanjima faze II nastavi pokazivati da je sigurno.

"Pokusi faze II bit će informativniji s obzirom na signal učinkovitosti", kaže Cooke, te dodaje da još puno koraka treba poduzeti i riješiti puno nepoznanica koje treba razmotriti prije nego što se bude moglo reći da smo pronašli lijek, a bilo koji od njih mogao bi nam pokazati da ovo cjepivo nije odgovor koji tražimo.
Razvoj sigurnog i djelotvornog cjepiva na mnogo je načina igra brojevima. Jedan određeni kandidat za cjepivo može ili ne mora biti siguran i učinkovit, ali ako nije, drugi bi mogao biti.
Dakle, iako su ovi prvi rezultati obećavajući,ali ako ovaj kandidat ne izdrži, onda ima razloga nadati se da će jedan od mnogo drugih koji se razvijaju biti uspješan.

Moderna, jedna od tvrtki koja radi na cjepivu protiv koronavirusa, objavila je najnovije rezultate ispitivanja faze I njihovog RNA cjepiva. Na prvi pogled vijest je uzbudljiva: cjepivo izgleda sigurno, a dobrovoljci su razvili antitijela.
No prije nego što svi počnemo planirati što ćemo raditi nakon što svi primimo cjepivo, važno je ove rezultate staviti u odgovarajući kontekst. Evo što biste trebali znati.
Ispitivanje faze I osmišljeno je kako bi se ispitala sigurnost, a ne učinkovitost. Naime, ispitivanje faze I prvenstveno ispituje koliko je cjepivo sigurno, a ne da li je učinkovito.
U ispitivanju faze I zdravi dobrovoljci dobivaju različite količine cjepiva i nadgledaju se zbog bilo kakvih nuspojava. U ovo ispitivanje uključeno je 45 zdravih dobrovoljaca koji su primili različite razine doze cjepiva.
Zasad se čini da je cjepivo sigurno, a FDA je odobrila prelazak na ispitivanja faze II, gdje će dobrovoljcima koji su u visokom riziku od virusa dati terapeutsku dozu.
Uz ispitivanje sigurnosti različitih doza, istraživači su također pratili dobrovoljce na znakove da je cjepivo potaknulo imunološki odgovor testiranjem kako bi utvrdili razvijaju li se antitijela protiv virusa.
Moderna je najavila da je od 45 osoba uključenih u ispitivanje osam moglo biti testirano, a njihova krv imala je očekivana neutralizirajuća antitijela na razinama "na ili iznad" onih kakvi su se razvili kod ljudi koji su se oporavili od COVID-19.
"Ovo su sjajne vijesti", kaže John Cooke, liječnik znanstvenik iz Houstonske metodističke bolnice, koji ne sudjeluje u istraživanju komentirao je: "Ali, još uvijek ne znamo hoće li stvaranje antitijela zaštititi ljude od bolesti."

To što se cjepivo pokazalo sigurnim za osam ljudi još ne znači da je sigurno za milijune ljudi. Tu je i pitanje koliko će ovo cjepivo biti učinkovito: Trenutno istraživači proučavaju stimulira li imunološki odgovor stvaranje antitijela slična onima pronađenim u oporavljenih pacijenata. Iako je ovo obećavajući rezultat, postoji nekoliko ograničenja.
Kako ističe Peter Hotez, istaknuti znanstvenik za cjepiva i stručnjak za tropske bolesti, posjedovanje antitijela protiv virusa nije nužno dovoljno.
Samo to što kod pacijenta možemo pronaći antitijela protiv virusa ne mora nužno značiti da su to bila antitijela koja su bila učinkovita u borbi protiv virusa, tako da još treba obaviti puno posla kako bi se utvrdilo koliko je ovo cjepivo zaista sigurno i učinkovito. Također je problematično što je ove rezultate objavila sama Moderna, umjesto u recenziranoj studiji.
Tu je i mogući sukob interesa. Naime, jedan od direktora Moderne, Moncef Slaoui, istupio je iz upravnog odbora i postao glavni znanstvenik za inicijativu Bijele kuće za ubrzavanje razvoja cjepiva. Slaoui trenutno ima više od 10 milijuna dolara dionica u Moderni. Otkako su ove vijesti objavljene, vrijednost njegovih dionica skočila je za oko 25 posto.
Činjenica da preliminarni dokazi pokazuju da ovo cjepivo izgleda sigurno i da može stimulirati imunološki sustav dobar je znak. Također je vrijedno napomenuti da je FDA odobrila ovom kandidatu za cjepivo da započne ispitivanja faze II, a ispitivanja faze III procjenjuju se da će započeti u srpnju, ukoliko se ispitivanjima faze II nastavi pokazivati da je sigurno.

"Pokusi faze II bit će informativniji s obzirom na signal učinkovitosti", kaže Cooke, te dodaje da još puno koraka treba poduzeti i riješiti puno nepoznanica koje treba razmotriti prije nego što se bude moglo reći da smo pronašli lijek, a bilo koji od njih mogao bi nam pokazati da ovo cjepivo nije odgovor koji tražimo.
Razvoj sigurnog i djelotvornog cjepiva na mnogo je načina igra brojevima. Jedan određeni kandidat za cjepivo može ili ne mora biti siguran i učinkovit, ali ako nije, drugi bi mogao biti.
Dakle, iako su ovi prvi rezultati obećavajući,ali ako ovaj kandidat ne izdrži, onda ima razloga nadati se da će jedan od mnogo drugih koji se razvijaju biti uspješan.
