Mnogi ljudi ne ostavljaju se duhana čak ni kad zbog njega teško obole, ne samo od raka, nego i od nekih drugih bolesti, kao što je kronična opstruktivna plućna bolest KOPB, emfizem... Takvih je primjera puno, među njima su neki moji kolege novinari, moja susjeda, poznanica mi je nedavno operirana radi začepljenja trbušne aorte, također kao posljedice pušenja itd.

Zašto je tako, otkrio nam je prof. dr. sc. Ante Bagarić, specijalist psihijatar, psihoterapeut i adiktolog, pročelnik Zavoda za liječenje ovisnosti na Klinici za psihijatriju "Vrapče".

– Duhan je teško ostaviti jer sadrži nikotin koji je psihoaktivna tvar i većina osoba koja puši već je razvila pravu ovisnost o duhanu i nikotinu (dijagnostička kategorija F17). Kao i kod drugih ovisnosti, ovisnost o nikotinu se razvija postupno, a kad se razvije, značajno utječe na sva važna životna područja ovisnika. Psihodinamski gledano, ovisnost o nikotinu je samo manjim dijelom hedonizam, sociopatija, samodestrukcija i slabost volje, ali je najvećim dijelom samo medikacija.

Nikotin, očito je, stvara prejaku ovisnost, ili je možda ipak stvar u karakteru?

– Točno je da nikotin sam po sebi stvara jaku ovisnost, ali u nastanku ovisnosti važnu ulogu imaju i karakteristike ličnosti koje su važna predispozicija za razvoj ovisnosti o nikotinu.

S druge strane, ima ljudi koji se, makar uz tešku muku, ipak ostave duhana, ali za njim strašno pate. Možete li reći što može biti tako jak motiv da se netko odrekne pušenja iako i dalje ima želju za pušenjem?

– Kao i kod svake ovisnosti, tako i kod ovisnosti o nikotinu nakon prestanka ili smanjenja uzimanja nikotina nastupaju apstinencijske smetnje. S vremenom one slabe i onda sasvim nestaju. Zbog toga je važno izdržati prvih nekoliko mjeseci. Osoba koja puši trebala bi svakodnevno razvijati i osnaživati sve motive koji dovode do prestanka pušenja (vlastito zdravlje, zdravlje bliskih osoba, financijska ušteda, jačanje vlastite ličnosti i slično).

Koliko ljudi koji se odluče na odvikavanje uopće uspiju, i imate li o tome neke podatke?

– Mark Twain je rekao: "Prestati pušiti je najlakša stvar. Ja sam to učinio već tisuću puta." Više od 80 posto osoba koje su prestale pušiti ponovno će početi pušiti u razdoblju od godine dana. No, taj podatak ne smije obeshrabriti pušače. Bilo koje razdoblje nepušenja je važno za osobu. I fizički, a pogotovo emocionalno. Dan, tjedan ili mjesec apstinencije osnažuju ličnost.

Dajte nam neke savjete koji mogu biti motivirajući da se ljudi odluče prestati pušiti, i zauvijek se riješe te za zdravlje pogubne navike?

– Osnovna komponenta svake ovisnosti je žudnja. Žudnja za psihoaktivnom tvari (kod duhana je to nikotin) osnovni je razlog za recidiv (ponovni početak pušenja). Za smanjenje žudnje vrlo je korisno znati da je žudnja uvijek prisutna i ne može se izbjeći; jačina žudnje se mijenja; okidači za pojačavanje žudnje su visoko individualizirani. Žudnja, također, jača kod jače izražene ovisnosti, dugotrajnije ovisnosti, ovisnosti o više psihoaktivnih tvari, više oštećenog funkcioniranja, komorbiditeta, mlađe osobe.

Žudnja je prirodna i ne treba je se sramiti ni osjećati krivnju zbog nje. Nije realan cilj biti bez žudnje, pravilan put je prepoznavati žudnju. Biti s drugim ovisnicima povećava žudnju, a povećava je i razgovor o pušenju. Nasuprot tome, razgovor o žudnji smanjuje žudnju; pomažu i posao, sport i zabava, kao i meditacija i relaksacija.

Karakteristike ličnosti i ovisnost

Karakteristike ličnosti koje čine važnu predispoziciju za razvoj ovisnosti o nikotinu:

- impulzivnost

- emocionalna nestabilnost

- protestiranje protiv autoriteta i društva uzimanjem psihoaktivnih tvari

- nepokoravanje socijalnim normama

- sklonost rizičnom ponašanju

- odbijanje preuzimanja odgovornosti

- izbjegavanje samostalnih aktivnosti

- ovisnost o podrški i pohvalama drugih

- potreba za bliskim odnosom

- submisivno ponašanje

- nemogućnost izražavanja neslaganja s drugim osobama

- teškoće u donošenju svakodnevnih odluka

- nemogućnost iskazivanja svojih potreba

- pasivno-agresivno reagiranje

- nestabilnost u interpersonalnim odnosima

- neprestano se žale na životne okolnosti

- fantazije o uspjehu

- smetnje identiteta

- poremećeno emocionalno, ljubavno i seksualno ponašanje

- sugestibilnost

- manipulativnost

- autodestruktivno ponašanje

- potreba da kažnjavaju i da budu kažnjeni

Mnogi ljudi ne ostavljaju se duhana čak ni kad zbog njega teško obole, ne samo od raka, nego i od nekih drugih bolesti, kao što je kronična opstruktivna plućna bolest KOPB, emfizem... Takvih je primjera puno, među njima su neki moji kolege novinari, moja susjeda, poznanica mi je nedavno operirana radi začepljenja trbušne aorte, također kao posljedice pušenja itd.

Zašto je tako, otkrio nam je prof. dr. sc. Ante Bagarić, specijalist psihijatar, psihoterapeut i adiktolog, pročelnik Zavoda za liječenje ovisnosti na Klinici za psihijatriju "Vrapče".

– Duhan je teško ostaviti jer sadrži nikotin koji je psihoaktivna tvar i većina osoba koja puši već je razvila pravu ovisnost o duhanu i nikotinu (dijagnostička kategorija F17). Kao i kod drugih ovisnosti, ovisnost o nikotinu se razvija postupno, a kad se razvije, značajno utječe na sva važna životna područja ovisnika. Psihodinamski gledano, ovisnost o nikotinu je samo manjim dijelom hedonizam, sociopatija, samodestrukcija i slabost volje, ali je najvećim dijelom samo medikacija.

Nikotin, očito je, stvara prejaku ovisnost, ili je možda ipak stvar u karakteru?

– Točno je da nikotin sam po sebi stvara jaku ovisnost, ali u nastanku ovisnosti važnu ulogu imaju i karakteristike ličnosti koje su važna predispozicija za razvoj ovisnosti o nikotinu.

S druge strane, ima ljudi koji se, makar uz tešku muku, ipak ostave duhana, ali za njim strašno pate. Možete li reći što može biti tako jak motiv da se netko odrekne pušenja iako i dalje ima želju za pušenjem?

– Kao i kod svake ovisnosti, tako i kod ovisnosti o nikotinu nakon prestanka ili smanjenja uzimanja nikotina nastupaju apstinencijske smetnje. S vremenom one slabe i onda sasvim nestaju. Zbog toga je važno izdržati prvih nekoliko mjeseci. Osoba koja puši trebala bi svakodnevno razvijati i osnaživati sve motive koji dovode do prestanka pušenja (vlastito zdravlje, zdravlje bliskih osoba, financijska ušteda, jačanje vlastite ličnosti i slično).

Koliko ljudi koji se odluče na odvikavanje uopće uspiju, i imate li o tome neke podatke?

– Mark Twain je rekao: "Prestati pušiti je najlakša stvar. Ja sam to učinio već tisuću puta." Više od 80 posto osoba koje su prestale pušiti ponovno će početi pušiti u razdoblju od godine dana. No, taj podatak ne smije obeshrabriti pušače. Bilo koje razdoblje nepušenja je važno za osobu. I fizički, a pogotovo emocionalno. Dan, tjedan ili mjesec apstinencije osnažuju ličnost.

Dajte nam neke savjete koji mogu biti motivirajući da se ljudi odluče prestati pušiti, i zauvijek se riješe te za zdravlje pogubne navike?

– Osnovna komponenta svake ovisnosti je žudnja. Žudnja za psihoaktivnom tvari (kod duhana je to nikotin) osnovni je razlog za recidiv (ponovni početak pušenja). Za smanjenje žudnje vrlo je korisno znati da je žudnja uvijek prisutna i ne može se izbjeći; jačina žudnje se mijenja; okidači za pojačavanje žudnje su visoko individualizirani. Žudnja, također, jača kod jače izražene ovisnosti, dugotrajnije ovisnosti, ovisnosti o više psihoaktivnih tvari, više oštećenog funkcioniranja, komorbiditeta, mlađe osobe.

Žudnja je prirodna i ne treba je se sramiti ni osjećati krivnju zbog nje. Nije realan cilj biti bez žudnje, pravilan put je prepoznavati žudnju. Biti s drugim ovisnicima povećava žudnju, a povećava je i razgovor o pušenju. Nasuprot tome, razgovor o žudnji smanjuje žudnju; pomažu i posao, sport i zabava, kao i meditacija i relaksacija.

Karakteristike ličnosti i ovisnost

Karakteristike ličnosti koje čine važnu predispoziciju za razvoj ovisnosti o nikotinu:

- impulzivnost

- emocionalna nestabilnost

- protestiranje protiv autoriteta i društva uzimanjem psihoaktivnih tvari

- nepokoravanje socijalnim normama

- sklonost rizičnom ponašanju

- odbijanje preuzimanja odgovornosti

- izbjegavanje samostalnih aktivnosti

- ovisnost o podrški i pohvalama drugih

- potreba za bliskim odnosom

- submisivno ponašanje

- nemogućnost izražavanja neslaganja s drugim osobama

- teškoće u donošenju svakodnevnih odluka

- nemogućnost iskazivanja svojih potreba

- pasivno-agresivno reagiranje

- nestabilnost u interpersonalnim odnosima

- neprestano se žale na životne okolnosti

- fantazije o uspjehu

- smetnje identiteta

- poremećeno emocionalno, ljubavno i seksualno ponašanje

- sugestibilnost

- manipulativnost

- autodestruktivno ponašanje

- potreba da kažnjavaju i da budu kažnjeni

Mnogi ljudi ne ostavljaju se duhana čak ni kad zbog njega teško obole, ne samo od raka, nego i od nekih drugih bolesti, kao što je kronična opstruktivna plućna bolest KOPB, emfizem... Takvih je primjera puno, među njima su neki moji kolege novinari, moja susjeda, poznanica mi je nedavno operirana radi začepljenja trbušne aorte, također kao posljedice pušenja itd.

Zašto je tako, otkrio nam je prof. dr. sc. Ante Bagarić, specijalist psihijatar, psihoterapeut i adiktolog, pročelnik Zavoda za liječenje ovisnosti na Klinici za psihijatriju "Vrapče".

– Duhan je teško ostaviti jer sadrži nikotin koji je psihoaktivna tvar i većina osoba koja puši već je razvila pravu ovisnost o duhanu i nikotinu (dijagnostička kategorija F17). Kao i kod drugih ovisnosti...
