Kašalj je jedan od najčešćih simptoma zbog kojih roditelji traže savjet kako liječnika primarne zdravstvene zaštite, tako i farmaceuta, a ujedno i čest razlog upućivanja djece na daljnju obradu u sekundarne i tercijarne zdravstvene ustanove. Shodno tome, potrebno je adekvatno prepoznati česta i bezazlena stanja kao uzrok kašlja kako se dijete ne bi nepotrebno podvrgavalo brojnim (i nerijetko kompliciranim) dijagnostičkim postupcima, javlja PlivaZdravlje. 

Za spomenuti portal savjete i objašnjenja je dao doc. dr. sc. Ivan Pavić, prim. dr. med., specijalist pedijatrije, subspecijalist pedijatrijske pulmologije.

Znanstveno je dokazano da kašalj značajno smanjuje kvalitetu života ne samo osobe koja kašlje, nego i njezinih ukućana. S druge strane, koliko god bio zabrinjavajući simptom za roditelje, u daleko najvećem broju slučajeva kašlja kod djece radi se o bezazlenom simptomu, odnosno o reakciji na akutne respiratorne infekcije (ARI), tj. pokušaju da se kašljem eliminira nakupljeni sekret iz dišnih putova. Dakle, radi se o pozitivnom refleksu koji je inače vrlo kompleksan te pod kontrolom centra u kralježničkoj moždini.

Kašalj podrazumijeva usklađenu aktivnost velikog broja respiracijskih (dišnih) mišića, a sam refleks kašlja inicira se podraživanjem receptora za kašalj koji se nalaze duž dišnog puta od ždrijela do bronhiola (mali dišni putevi koji se nastavljaju na bronhe i provode zrak do alveola), ali i izvan grudnog koša. Receptori za kašalj mogu reagirati na dodir, iritaciju uslijed postojanja upale i na kemijske podražaje. Alveole kao najsitniji dio dišnog puta nemaju receptore za kašalj, tako da kašalj neće biti simptom upala s primarnim zahvaćanjem alveola. Važno je naglasiti kako je utvrđeno da djeca bez ikakve prisutne bolesti mogu u prosjeku zakašljati do jedanaest puta dnevno.

Učestalost i duljina trajanja kašlja

Kao što je već naglašeno kašalj kod djece se najčešće javlja u tijeku ARI, koje su i inače jedan od vodećih uzroka pobola u dječjoj dobi – poglavito u djece do četvrte godine života. Postoji nekoliko važnih razloga velike učestalosti ARI: dišni sustav je najizloženiji organski sustav koji je u neprestanom kontaktu s okolinom, izuzetno je lak način prijenosa infekcija, a uslijed velikog broja potencijalnih uzročnika bolesti mogućnosti njihova sprečavanja su vrlo slabe u svakodnevnoj praksi. Iako se navedeno jednako odnosi na sve dobne skupine, dišni sustav u djece još je podložniji respiratornim infekcijama, prvenstveno zbog razvojne fiziologije dišnog i imunološkog sustava  u toj dobi (a što je i uzrok češće pojavnosti kašlja kod djece).

Nadalje, s obzirom na duljinu trajanja, kašalj u djece se najčešće dijeli na akutni (koji traje do četiri tjedna) i kronični (koji traje preko četiri tjedna). S druge strane, uzimajući u obzir relevantna epidemiološka istraživanja koja su pokazala da kašalj u djece posljedično respiratornim infekcijama kao najčešćeg uzroka kašlja može trajati i dulje od četiri tjedna, ovisno o stupnju oštećenja i trajanju oporavka respiratornog epitela, neke smjernice (primjerice, u Velikoj Britaniji) definiraju kronični kašalj kod djece kao i kod odraslih, kao onaj koji traje dulje od osam tjedana. U tom slučaju kašalj koji traje od 4-8 tjedana se definira kao prolongirani subakutni kašalj, a pokazalo se kako ovakva podjela dobro koincidira s uzrokom kašlja. 

Liječenje 

Kašalj u djece mora biti liječen etiološki tj. uzročno, tako da se djeci ne preporučuje ordinirati lijekove za uklanjanje kašlja prije negoli se utvrdi njegov uzrok. Dok simptomatska terapija kod akutnog kašlja traje do desetak dana, kod kroničnog kašlja se mora primijeniti terapija koja djeluje na uzročni faktor. Kad se utvrdi uzrok kroničnog kašlja kod djeteta, terapija se provodi prema principima liječenja osnovne bolesti koja ga uzrokuje; na taj način će liječenje osnovne bolesti dovesti do znatnog smanjenja, odnosno nestanka kroničnog kašlja kod djeteta. Kod sve djece s kroničnim kašljem obvezna je rendgenska snimka pluća, a ako se na snimci pluća utvrdi postojanje promjena, neophodno je dodatno ispitivanje radi utvrđivanja bolesti koja ih je izazvala.

Kašalj je jedan od najčešćih simptoma zbog kojih roditelji traže savjet kako liječnika primarne zdravstvene zaštite, tako i farmaceuta, a ujedno i čest razlog upućivanja djece na daljnju obradu u sekundarne i tercijarne zdravstvene ustanove. Shodno tome, potrebno je adekvatno prepoznati česta i bezazlena stanja kao uzrok kašlja kako se dijete ne bi nepotrebno podvrgavalo brojnim (i nerijetko kompliciranim) dijagnostičkim postupcima, javlja PlivaZdravlje. 

Za spomenuti portal savjete i objašnjenja je dao doc. dr. sc. Ivan Pavić, prim. dr. med., specijalist pedijatrije, subspecijalist pedijatrijske pulmologije.

Znanstveno je dokazano da kašalj značajno smanjuje kvalitetu života ne samo osobe koja kašlje, nego i njezinih ukućana. S druge strane, koliko god bio zabrinjavajući simptom za roditelje, u daleko najvećem broju slučajeva kašlja kod djece radi se o bezazlenom simptomu, odnosno o reakciji na akutne respiratorne infekcije (ARI), tj. pokušaju da se kašljem eliminira nakupljeni sekret iz dišnih putova. Dakle, radi se o pozitivnom refleksu koji je inače vrlo kompleksan te pod kontrolom centra u kralježničkoj moždini.

Kašalj podrazumijeva usklađenu aktivnost velikog broja respiracijskih (dišnih) mišića, a sam refleks kašlja inicira se podraživanjem receptora za kašalj koji se nalaze duž dišnog puta od ždrijela do bronhiola (mali dišni putevi koji se nastavljaju na bronhe i provode zrak do alveola), ali i izvan grudnog koša. Receptori za kašalj mogu reagirati na dodir, iritaciju uslijed postojanja upale i na kemijske podražaje. Alveole kao najsitniji dio dišnog puta nemaju receptore za kašalj, tako da kašalj neće biti simptom upala s primarnim zahvaćanjem alveola. Važno je naglasiti kako je utvrđeno da djeca bez ikakve prisutne bolesti mogu u prosjeku zakašljati do jedanaest puta dnevno.

Učestalost i duljina trajanja kašlja

Kao što je već naglašeno kašalj kod djece se najčešće javlja u tijeku ARI, koje su i inače jedan od vodećih uzroka pobola u dječjoj dobi – poglavito u djece do četvrte godine života. Postoji nekoliko važnih razloga velike učestalosti ARI: dišni sustav je najizloženiji organski sustav koji je u neprestanom kontaktu s okolinom, izuzetno je lak način prijenosa infekcija, a uslijed velikog broja potencijalnih uzročnika bolesti mogućnosti njihova sprečavanja su vrlo slabe u svakodnevnoj praksi. Iako se navedeno jednako odnosi na sve dobne skupine, dišni sustav u djece još je podložniji respiratornim infekcijama, prvenstveno zbog razvojne fiziologije dišnog i imunološkog sustava  u toj dobi (a što je i uzrok češće pojavnosti kašlja kod djece).

Nadalje, s obzirom na duljinu trajanja, kašalj u djece se najčešće dijeli na akutni (koji traje do četiri tjedna) i kronični (koji traje preko četiri tjedna). S druge strane, uzimajući u obzir relevantna epidemiološka istraživanja koja su pokazala da kašalj u djece posljedično respiratornim infekcijama kao najčešćeg uzroka kašlja može trajati i dulje od četiri tjedna, ovisno o stupnju oštećenja i trajanju oporavka respiratornog epitela, neke smjernice (primjerice, u Velikoj Britaniji) definiraju kronični kašalj kod djece kao i kod odraslih, kao onaj koji traje dulje od osam tjedana. U tom slučaju kašalj koji traje od 4-8 tjedana se definira kao prolongirani subakutni kašalj, a pokazalo se kako ovakva podjela dobro koincidira s uzrokom kašlja. 

Liječenje 

Kašalj u djece mora biti liječen etiološki tj. uzročno, tako da se djeci ne preporučuje ordinirati lijekove za uklanjanje kašlja prije negoli se utvrdi njegov uzrok. Dok simptomatska terapija kod akutnog kašlja traje do desetak dana, kod kroničnog kašlja se mora primijeniti terapija koja djeluje na uzročni faktor. Kad se utvrdi uzrok kroničnog kašlja kod djeteta, terapija se provodi prema principima liječenja osnovne bolesti koja ga uzrokuje; na taj način će liječenje osnovne bolesti dovesti do znatnog smanjenja, odnosno nestanka kroničnog kašlja kod djeteta. Kod sve djece s kroničnim kašljem obvezna je rendgenska snimka pluća, a ako se na snimci pluća utvrdi postojanje promjena, neophodno je dodatno ispitivanje radi utvrđivanja bolesti koja ih je izazvala.

Kašalj je jedan od najčešćih simptoma zbog kojih roditelji traže savjet kako liječnika primarne zdravstvene zaštite, tako i farmaceuta, a ujedno i čest razlog upućivanja djece na daljnju obradu u sekundarne i tercijarne zdravstvene ustanove. Shodno tome, potrebno je adekvatno prepoznati česta i bezazlena stanja kao uzrok kašlja kako se dijete ne bi nepotrebno podvrgavalo brojnim (i nerijetko kompliciranim) dijagnostičkim postupcima, javlja PlivaZdravlje.
