EMA razmatra nuspojavu tromboze kod cijepljenih Johnson&Johnsonom koji je iste tehnologije kao i AstraZeneca i Sputnjik V

Baš u trenutku kad se očekuju veće doze cjepiva u zemljama EU, pa i u Hrvatskoj, interes građana za cijepljenje dijelom je splasnuo, čemu su doprinijele i globalne rasprave o cjepivu AstraZenece i njegove moguće povezanosti s tromboembolijama.

Novi problem mogao bi biti izazvan vijestima koje stižu iz Europske agencije za lijekove (EMA), ali ovaj put vezane uz cjepivo Johnson&Johnson jer postoje sumnje da je i kod njega jedna od rijetkih nuspojava tromboembolija.

Aktivnosti u krvi

EMA je ovih dana razmatrala tri slučaja neobičnih krvnih ugrušaka kod cijepljenih Johnson&Johnsonovim cjepivom koje će sljedeći tjedan stići u zemlje EU. Odbor za procjenu farmakovigilancije EMA-e izvijestio je da su zabilježena četiri slučaja tromboembolije, od kojih je jedan registriran tijekom kliničkih ispitivanja, a iz tvrtke kažu kako je njihovo praćenje nuspojava otkrilo vrlo mali broj rijetkih događaja nakon cijepljenja.

Cjepivo Johnson&Johnson, koje se daje u samo jednoj dozi, također je vektorsko, odnosno iste tehnologije kao i AstraZenecino, a razlikuju se samo po adenovirusima koje koriste kao vektor. Znači li to da bi onda sva vektorska cjepiva, uključujući i ruski Sputnjik V, mogla imati slične rijetke nuspojave, pitaju se oni koji čekaju na jedno od četiri registrirana cjepiva u EU.

"Ova događanja, koja se povezuju s AstraZenecinim cjepivom, zasad nisu dobila snažnije utemeljenje i bit će nužno usporediti moguće nuspojave ovog cjepiva s ostalim adenovirusnim cjepivima kod kojih se koristi ista tehnologija. Dok Johnson&Johnson koristi humani adenovirusni vektor AD 26, AstraZeneca koristi kao vektor adenovirus čimpanze. Sputnjik V također koristi AD 26. Sve su te vakcine temeljene na dijelu spike proteina koji nosi vektor pa se vjerojatno ne radi o reakciji na taj protein, ali moguće je da se radi o odgovoru na različite adenovirusne proteine. Moguće je da se u tom slučaju pojavi neka aktivnost trombocita. No, to je samo nagađanje jer nema dokaza da se u tim slučajevima radi o vezivanju antitijela na trombocite", objašnjava prof. dr. Bojan Polić sa Zavoda za histologiju i embriologiju Medicinskog fakulteta u Rijeci.

Naime, nakon što su se pojavile informacije o tromboemboličkim događajima kod cijepljenih, znanstvenici su postavili hipotezu da nakon primitka cjepiva, zbog razloga koji tek treba utvrditi, neki ljudi počnu stvarati antitijela koja se zatim povežu na određeni trombocitni protein, nazvan trombocitni faktor 4.

Paradoks u tijelu

To vezanje antitijela zbuni imunološki sustav pa on počne napadati vlastite trombocite, smanjuje im broj i potiče ih na otpuštanje koktela proteina među kojima se nalazi i trombin koji izaziva zgrušavanje krvi.

Tako dolazi do paradoksalne situacije da u krvotoku raste rizik od zgrušavanja, a istodobno se smanjuje broj trombocita koji inače izazivaju zgrušavanje. No, prof. dr. Polić smatra da će biti nužno pratiti i uspoređivati što se događa s cijepljenima AstraZenecom odnosno Johnson&Johnsonom u odnosu na one cijepljene mRNA cjepivom i vidjeti postoje li mogući tromboembolički događaj kod obje vrste cjepiva ili samo kod adenovirusnih.

"Pokaže li se da razlika postoji, onda bi se moglo posumnjati da se radi o nekom imunološkom odgovoru na adenovirusne proteine. No, čvrstih dokaza za to zasad nema, ali čak i da postoje, cijepljenje je manje opasno od zaraze koronom", ističe prof. dr. Polić.

Dodaje da su tromboembolička događanja učestala u populaciji. Čak 30 do 50 posto pacijenata s covidom ima tromboembolijske probleme, a kod teških slučajeva još su češći. Cijepljenje je jedini izlaz iz epidemije, a rizik od nuspojava gotovo je minoran.

EMA razmatra nuspojavu tromboze kod cijepljenih Johnson&Johnsonom koji je iste tehnologije kao i AstraZeneca i Sputnjik V

Baš u trenutku kad se očekuju veće doze cjepiva u zemljama EU, pa i u Hrvatskoj, interes građana za cijepljenje dijelom je splasnuo, čemu su doprinijele i globalne rasprave o cjepivu AstraZenece i njegove moguće povezanosti s tromboembolijama.

Novi problem mogao bi biti izazvan vijestima koje stižu iz Europske agencije za lijekove (EMA), ali ovaj put vezane uz cjepivo Johnson&Johnson jer postoje sumnje da je i kod njega jedna od rijetkih nuspojava tromboembolija.

Aktivnosti u krvi

EMA je ovih dana razmatrala tri slučaja neobičnih krvnih ugrušaka kod cijepljenih Johnson&Johnsonovim cjepivom koje će sljedeći tjedan stići u zemlje EU. Odbor za procjenu farmakovigilancije EMA-e izvijestio je da su zabilježena četiri slučaja tromboembolije, od kojih je jedan registriran tijekom kliničkih ispitivanja, a iz tvrtke kažu kako je njihovo praćenje nuspojava otkrilo vrlo mali broj rijetkih događaja nakon cijepljenja.

Cjepivo Johnson&Johnson, koje se daje u samo jednoj dozi, također je vektorsko, odnosno iste tehnologije kao i AstraZenecino, a razlikuju se samo po adenovirusima koje koriste kao vektor. Znači li to da bi onda sva vektorska cjepiva, uključujući i ruski Sputnjik V, mogla imati slične rijetke nuspojave, pitaju se oni koji čekaju na jedno od četiri registrirana cjepiva u EU.

"Ova događanja, koja se povezuju s AstraZenecinim cjepivom, zasad nisu dobila snažnije utemeljenje i bit će nužno usporediti moguće nuspojave ovog cjepiva s ostalim adenovirusnim cjepivima kod kojih se koristi ista tehnologija. Dok Johnson&Johnson koristi humani adenovirusni vektor AD 26, AstraZeneca koristi kao vektor adenovirus čimpanze. Sputnjik V također koristi AD 26. Sve su te vakcine temeljene na dijelu spike proteina koji nosi vektor pa se vjerojatno ne radi o reakciji na taj protein, ali moguće je da se radi o odgovoru na različite adenovirusne proteine. Moguće je da se u tom slučaju pojavi neka aktivnost trombocita. No, to je samo nagađanje jer nema dokaza da se u tim slučajevima radi o vezivanju antitijela na trombocite", objašnjava prof. dr. Bojan Polić sa Zavoda za histologiju i embriologiju Medicinskog fakulteta u Rijeci.

Naime, nakon što su se pojavile informacije o tromboemboličkim događajima kod cijepljenih, znanstvenici su postavili hipotezu da nakon primitka cjepiva, zbog razloga koji tek treba utvrditi, neki ljudi počnu stvarati antitijela koja se zatim povežu na određeni trombocitni protein, nazvan trombocitni faktor 4.

Paradoks u tijelu

To vezanje antitijela zbuni imunološki sustav pa on počne napadati vlastite trombocite, smanjuje im broj i potiče ih na otpuštanje koktela proteina među kojima se nalazi i trombin koji izaziva zgrušavanje krvi.

Tako dolazi do paradoksalne situacije da u krvotoku raste rizik od zgrušavanja, a istodobno se smanjuje broj trombocita koji inače izazivaju zgrušavanje. No, prof. dr. Polić smatra da će biti nužno pratiti i uspoređivati što se događa s cijepljenima AstraZenecom odnosno Johnson&Johnsonom u odnosu na one cijepljene mRNA cjepivom i vidjeti postoje li mogući tromboembolički događaj kod obje vrste cjepiva ili samo kod adenovirusnih.

"Pokaže li se da razlika postoji, onda bi se moglo posumnjati da se radi o nekom imunološkom odgovoru na adenovirusne proteine. No, čvrstih dokaza za to zasad nema, ali čak i da postoje, cijepljenje je manje opasno od zaraze koronom", ističe prof. dr. Polić.

Dodaje da su tromboembolička događanja učestala u populaciji. Čak 30 do 50 posto pacijenata s covidom ima tromboembolijske probleme, a kod teških slučajeva još su češći. Cijepljenje je jedini izlaz iz epidemije, a rizik od nuspojava gotovo je minoran.

Pod povećalom Agencije za lijekove i cjepivo Johnson&Johnson koje idući tjedan stiže u EU: Problem su opet krvni ugrušci

EMA razmatra nuspojavu tromboze kod cijepljenih Johnson&Johnsonom koji je iste tehnologije kao i AstraZeneca i Sputnjik V

Baš u trenutku kad se očekuju veće doze cjepiva u zemljama EU, pa i u Hrvatskoj, interes građana za cijepljenje dijelom je splasnuo, čemu su doprinijele i globalne rasprave o cjepivu AstraZenece i njegove moguće povezanosti s tromboembolijama.

Novi problem mogao bi biti izazvan vijestima koje stižu iz Europske agencije za lijekove (EMA), ali ovaj put vezane uz cjepivo Johnson&Johnson jer postoje sumnje da je i kod njega jedna od rijetkih nuspojava tromboembolija.

Aktivnosti u krvi

EMA je ovih dana razmatrala tri slučaja neobičnih krvnih ugrušaka kod cijepljenih Johnson&Johnsonovim cjepivom koje će sljedeći tjedan stići u zemlje EU. Odbor za procjenu farmakovigilancije EMA-e izvijestio je da su zabilježena četiri slučaja tromboembolije, od kojih je jedan registriran tijekom kliničkih ispitivanja, a iz tvrtke kažu kako je njihovo praćenje nuspojava otkrilo vrlo mali broj rijetkih događaja nakon cijepljenja.

Cjepivo Johnson&Johnson, koje se daje u samo jednoj dozi, također je vektorsko, odnosno iste tehnologije kao i AstraZenecino, a razlikuju se samo po adenovirusima koje koriste kao vektor. Znači li to da bi onda sva vektorska cjepiva, uključujući i ruski Sputnjik V, mogla imati slične rijetke nuspojave, pitaju se oni koji čekaju na jedno od četiri registrirana cjepiva u EU.
