Poznato je da autoimune bolesti češće pogađaju žene nego muškarce. Autoimuna bolest je stanje u kojem imunološki sustav pojedinca zamjenjuje normalne stanice stranim i stoga ih napada.

Na primjer, dijabetes tipa 1 dijagnosticira se kada imunološki sustav napadne gušteraču. Drugi primjeri su: sustavni eritematozni lupus koji zahvaća cijelo tijelo; reumatoidni artritis koji zahvaća zglobove; psorijaza koja utječe na stanice kože; upalna bolest crijeva koja zahvaća crijeva; celijakija koja pogađa gastrointestinalni trakt.

Zdravstveni stručnjaci nisu uspjeli utvrditi točne okidače ili razloge ovih lažnih napada imunološkog sustava.

Koja je uloga posteljice?

Evolucijska biologinja Melissa A. Wilson i njezine kolege sa Sveučilišta Arizona State objavili su članak u kojem se raspravlja o hipotezi o povezanosti trudnoće i autoimune bolesti.

Teorija pretpostavlja da je ženski imunološki sustav povezan s posteljicom. Posteljica je organ u kojem se odvija metabolizam između majčine i fetalne krvi. Kako posteljica trudnice raste, njezini organi govore imunološkom sustavu da mora prihvatiti posteljicu i fetus. Govore tijelu, a time i imunološkom sustavu, da ih ne treba tretirati i odbaciti kao strano tijelo.

Što kažu teorije?

To je ono što teorija kaže: žene su se razvile na takav način da mogu biti trudne velik dio svog odraslog života. Prije uvođenja kontracepcije, to je bila sudbina ženskog spola. Žene su imale osam do dvanaest djece. Iako bi rođenje tolikog broja beba moglo zvučati stresno, žensko tijelo razvilo se do te mjere da su se uspjele nositi s tim.

Kako posteljica raste tijekom trudnoće, organ šalje signale imunološkom sustavu majke da promijeni svoju aktivnost kako majčino tijelo ne bi odbacilo posteljicu i fetus. To čak može značiti slom imunološkog sustava na različite načine ili u različito vrijeme. Premali imunitet čini tijelo osjetljivim na patogene koji mogu naštetiti fetusu.

Wilson i njezin tim vjeruju da imunološki sustav to nadoknađuje na druge načine. U nekim razdobljima smanjena funkcija imunološkog sustava dovodi do toga da imunološki sustav kasnije funkcionira previše.

Međutim, stvari se kompliciraju kada se te trudnoće zapravo ne dogode. Danas žene imaju daleko manje djece - u prosjeku dvoje u razvijenom svijetu.

Milijunima godina očekivalo se da će imunološki sustav biti izložen posteljici. U posljednjih 100 godina, s manje trudnoća, stvari su se promijenile. Kao da je netko cijelo vrijeme vukao nešto teško, pa kada bi naglo popustio pao bi na pod.

Imunitet postaje agresivan i nestabilan

Stoga imunološki sustav može postati previše agresivan, previše nestabilan. Također može početi napadati ono što ne šteti tijelu i kada nema opasnosti. Tako počinju autoimune bolesti.

Postoje mnoge druge teorije o tome zašto žene češće od muškaraca pogađaju autoimune bolesti. Čini se da teorija Wilsonove pokriva mnoge od njih i pruža dodatno objašnjenje koje uključuje evoluciju.

Trebalo bi učiniti još testova

Do sada se nitko nije suprotstavio njenoj teoriji, što nije čudno.

"Rekao bih da ne postoji jedna teorija koja objašnjava sve autoimune bolesti", kaže Nikolaos Patsopoulos, docent neurologije u Brigham and Women’s Hospital.

On priznaje, međutim, da "Wilson teorija kombinira mnoge činjenice za koje znamo da su istinite i neke koje još uvijek pokušavamo razumjeti."

Međutim, nisu svi oduševljeni ovom teorijom. Iako je to zanimljiva ideja, ipak ju je potrebno testirati.

David Hafler, profesor neurologije na Medicinskom fakultetu Yale, kaže:

“Ideje su jednostavne. Teško je doći do podataka.”

Wilson: Moj rad ima izravan utjecaj

Srećom, to možemo provjeriti. Znanstvenici mogu pratiti mnoge trudnoće i predvidjeti imaju li rizik od razvoja autoimune bolesti.

Ako je teorija istinita, žene s višestrukim trudnoćama imaju manji rizik.

"Uzbuđena sam", kaže Wilson. "Ovo je prvi put da mogu vidjeti da moj rad ima izravan utjecaj na zdravlje ljudi u sljedećih 10 godina."

Poznato je da autoimune bolesti češće pogađaju žene nego muškarce. Autoimuna bolest je stanje u kojem imunološki sustav pojedinca zamjenjuje normalne stanice stranim i stoga ih napada.

Na primjer, dijabetes tipa 1 dijagnosticira se kada imunološki sustav napadne gušteraču. Drugi primjeri su: sustavni eritematozni lupus koji zahvaća cijelo tijelo; reumatoidni artritis koji zahvaća zglobove; psorijaza koja utječe na stanice kože; upalna bolest crijeva koja zahvaća crijeva; celijakija koja pogađa gastrointestinalni trakt.

Zdravstveni stručnjaci nisu uspjeli utvrditi točne okidače ili razloge ovih lažnih napada imunološkog sustava.

Koja je uloga posteljice?

Evolucijska biologinja Melissa A. Wilson i njezine kolege sa Sveučilišta Arizona State objavili su članak u kojem se raspravlja o hipotezi o povezanosti trudnoće i autoimune bolesti.

Teorija pretpostavlja da je ženski imunološki sustav povezan s posteljicom. Posteljica je organ u kojem se odvija metabolizam između majčine i fetalne krvi. Kako posteljica trudnice raste, njezini organi govore imunološkom sustavu da mora prihvatiti posteljicu i fetus. Govore tijelu, a time i imunološkom sustavu, da ih ne treba tretirati i odbaciti kao strano tijelo.

Što kažu teorije?

To je ono što teorija kaže: žene su se razvile na takav način da mogu biti trudne velik dio svog odraslog života. Prije uvođenja kontracepcije, to je bila sudbina ženskog spola. Žene su imale osam do dvanaest djece. Iako bi rođenje tolikog broja beba moglo zvučati stresno, žensko tijelo razvilo se do te mjere da su se uspjele nositi s tim.

Kako posteljica raste tijekom trudnoće, organ šalje signale imunološkom sustavu majke da promijeni svoju aktivnost kako majčino tijelo ne bi odbacilo posteljicu i fetus. To čak može značiti slom imunološkog sustava na različite načine ili u različito vrijeme. Premali imunitet čini tijelo osjetljivim na patogene koji mogu naštetiti fetusu.

Wilson i njezin tim vjeruju da imunološki sustav to nadoknađuje na druge načine. U nekim razdobljima smanjena funkcija imunološkog sustava dovodi do toga da imunološki sustav kasnije funkcionira previše.

Međutim, stvari se kompliciraju kada se te trudnoće zapravo ne dogode. Danas žene imaju daleko manje djece - u prosjeku dvoje u razvijenom svijetu.

Milijunima godina očekivalo se da će imunološki sustav biti izložen posteljici. U posljednjih 100 godina, s manje trudnoća, stvari su se promijenile. Kao da je netko cijelo vrijeme vukao nešto teško, pa kada bi naglo popustio pao bi na pod.

Imunitet postaje agresivan i nestabilan

Stoga imunološki sustav može postati previše agresivan, previše nestabilan. Također može početi napadati ono što ne šteti tijelu i kada nema opasnosti. Tako počinju autoimune bolesti.

Postoje mnoge druge teorije o tome zašto žene češće od muškaraca pogađaju autoimune bolesti. Čini se da teorija Wilsonove pokriva mnoge od njih i pruža dodatno objašnjenje koje uključuje evoluciju.

Trebalo bi učiniti još testova

Do sada se nitko nije suprotstavio njenoj teoriji, što nije čudno.

"Rekao bih da ne postoji jedna teorija koja objašnjava sve autoimune bolesti", kaže Nikolaos Patsopoulos, docent neurologije u Brigham and Women’s Hospital.

On priznaje, međutim, da "Wilson teorija kombinira mnoge činjenice za koje znamo da su istinite i neke koje još uvijek pokušavamo razumjeti."

Međutim, nisu svi oduševljeni ovom teorijom. Iako je to zanimljiva ideja, ipak ju je potrebno testirati.

David Hafler, profesor neurologije na Medicinskom fakultetu Yale, kaže:

“Ideje su jednostavne. Teško je doći do podataka.”

Wilson: Moj rad ima izravan utjecaj

Srećom, to možemo provjeriti. Znanstvenici mogu pratiti mnoge trudnoće i predvidjeti imaju li rizik od razvoja autoimune bolesti.

Ako je teorija istinita, žene s višestrukim trudnoćama imaju manji rizik.

"Uzbuđena sam", kaže Wilson. "Ovo je prvi put da mogu vidjeti da moj rad ima izravan utjecaj na zdravlje ljudi u sljedećih 10 godina."
