Testiranje na COVID-19 nije uvjet za pedijatrijski pregled febrilne djece, provjerili smo u Domu zdravlja Dubrovnik i Općoj bolnici. Iz Ministarstva zdravstva, Stožera civilne zaštite, Zavoda za javno zdravstvo ili uprava ovih zdravstvenih ustanova, nikada nije upućena takva preporuka da se djecu prije pregleda prvo šalje na PCR testiranje, kažu sugovornici.

- Ne, ne i ne, to nikako nije preporuka! Svako dijete mora biti primljeno na pregled. Provjerio sam prije 15-ak dana, u pedijatrijskim ordinacijama Doma zdravlja Dubrovnik nitko nije vratio dijete zbog toga što nije imalo PCR test – kaže Branko Bazdan, ravnatelj Doma zdravlja Dubrovnik.

Apsurdno

Naime, na društvenim mrežama je iznesen jedan takav slučaj, u medijima prenesen anonimno. Štogod se dogodilo, roditelji trebaju znati da ambulante imaju izdvojene dijelove za zaraznu djecu i da, kako kaže Bazdan, nije logično tražiti testiranje djeteta da bi uopće došlo do pedijatra na pregled.

- Takvu apsurdnu preporuku nismo dobili niti smo se na taj način ponašali. Kod nas dolaze pacijenti i bez uputnica, na indikaciju roditelja, što je stari problem. Gospar Bazdan zna da subotu popodne i čitavu nedjelju nema organiziranu primarnu zdravstvenu zaštitu skrbi za djecu. To je puno veći problem od bilo kakve COVID potvrde ili nekakvoga brisa, ali narod na to ne diže galamu jer zna da pregled mogu obaviti kod nas. A, što se dogodilo neki dan: liječnica je bila na carskom rezu, doveli su COVID pozitivno dijete, u ambulantu je ušlo dijete s epileptičkim napadom, a ispred respiratorne ambulante urla dijete s povišenom temperaturom, sve na jednog doktora. Nakon COVID pacijenta trebate se ventat' bar 15 minuta, ne što biste htjeli, nego što morate! Vječno ponavljam, primarna zaštita se mora organizirati na način da, kao i u svim drugim gradovima u Hrvatskoj, imaju dežurstva 24 sata, jer za neke obične stvari ne mora biti pedijatar specijalist u dežurstvu. To u Dubrovniku ne prolazi – komentira prim. dr. Marija Radonić, voditeljica Odjela za pedijatriju.

Moraš dijete pregledati

Kako to u pandemijskim uvjetima izgleda kad dovodite dijete na pregled u Opću bolnicu Dubrovnik? Na dijelu hitnog prijama postoji odvojena ambulanta za COVID pozitivne pacijente s posebnim ulazom. Sva febrilna djeca koja nemaju potvrđeni koronavirus ulaze na drugi, također izdvojeni ulaz, s vanjske strane pedijatrijskog odjela, iz vrta. Tu je uređen prilaz u tzv. respiratornu ambulantu za svu djecu s povišenom tjelesnom temperaturom. U hodniku pokraj bolničke recepcije, odnosno u 'staroj' pedijatrijskoj čekaonici, na pregled čekaju sva ostala afebrilna djeca, kako bi se izbjegli mogući kontakti sa zaraženim pacijentima. Ni u bolnici nema uvjetovanja pregleda akutno bolesnog djeteta PCR testom.

- To nije preporuka niti je praksa. Moraš dijete pregledati, vidjeti što mu je, uostalom gdje će napraviti PCR? Ne može se pregled uvjetovati testom. Takva preporuka nije došla iz državne ni lokalne institucije, od ministra, ravnatelja, Zavoda za javno zdravstvo niti mene kao šefa službe, jer je to apsurd. PCR test tražimo samo kad dijete legne u bolnicu i za djecu stariju od sedam godina koja dolaze na spirometriju. Dakle, PCR se ne traži za pregled nego dijagnostiku u ovom slučaju, jer imamo samo jedan spirometar u kojeg pušu sva djeca i stoga se želi ukloniti rizik. To su moji kroničari s kojima imam dobru komunikaciju i oni to znaju unaprijed – rekla je prim. dr. Radonić.

Testiranje na COVID-19 nije uvjet za pedijatrijski pregled febrilne djece, provjerili smo u Domu zdravlja Dubrovnik i Općoj bolnici. Iz Ministarstva zdravstva, Stožera civilne zaštite, Zavoda za javno zdravstvo ili uprava ovih zdravstvenih ustanova, nikada nije upućena takva preporuka da se djecu prije pregleda prvo šalje na PCR testiranje, kažu sugovornici.

- Ne, ne i ne, to nikako nije preporuka! Svako dijete mora biti primljeno na pregled. Provjerio sam prije 15-ak dana, u pedijatrijskim ordinacijama Doma zdravlja Dubrovnik nitko nije vratio dijete zbog toga što nije imalo PCR test – kaže Branko Bazdan, ravnatelj Doma zdravlja Dubrovnik.

Apsurdno

Naime, na društvenim mrežama je iznesen jedan takav slučaj, u medijima prenesen anonimno. Štogod se dogodilo, roditelji trebaju znati da ambulante imaju izdvojene dijelove za zaraznu djecu i da, kako kaže Bazdan, nije logično tražiti testiranje djeteta da bi uopće došlo do pedijatra na pregled.

- Takvu apsurdnu preporuku nismo dobili niti smo se na taj način ponašali. Kod nas dolaze pacijenti i bez uputnica, na indikaciju roditelja, što je stari problem. Gospar Bazdan zna da subotu popodne i čitavu nedjelju nema organiziranu primarnu zdravstvenu zaštitu skrbi za djecu. To je puno veći problem od bilo kakve COVID potvrde ili nekakvoga brisa, ali narod na to ne diže galamu jer zna da pregled mogu obaviti kod nas. A, što se dogodilo neki dan: liječnica je bila na carskom rezu, doveli su COVID pozitivno dijete, u ambulantu je ušlo dijete s epileptičkim napadom, a ispred respiratorne ambulante urla dijete s povišenom temperaturom, sve na jednog doktora. Nakon COVID pacijenta trebate se ventat' bar 15 minuta, ne što biste htjeli, nego što morate! Vječno ponavljam, primarna zaštita se mora organizirati na način da, kao i u svim drugim gradovima u Hrvatskoj, imaju dežurstva 24 sata, jer za neke obične stvari ne mora biti pedijatar specijalist u dežurstvu. To u Dubrovniku ne prolazi – komentira prim. dr. Marija Radonić, voditeljica Odjela za pedijatriju.

Moraš dijete pregledati

Kako to u pandemijskim uvjetima izgleda kad dovodite dijete na pregled u Opću bolnicu Dubrovnik? Na dijelu hitnog prijama postoji odvojena ambulanta za COVID pozitivne pacijente s posebnim ulazom. Sva febrilna djeca koja nemaju potvrđeni koronavirus ulaze na drugi, također izdvojeni ulaz, s vanjske strane pedijatrijskog odjela, iz vrta. Tu je uređen prilaz u tzv. respiratornu ambulantu za svu djecu s povišenom tjelesnom temperaturom. U hodniku pokraj bolničke recepcije, odnosno u 'staroj' pedijatrijskoj čekaonici, na pregled čekaju sva ostala afebrilna djeca, kako bi se izbjegli mogući kontakti sa zaraženim pacijentima. Ni u bolnici nema uvjetovanja pregleda akutno bolesnog djeteta PCR testom.

- To nije preporuka niti je praksa. Moraš dijete pregledati, vidjeti što mu je, uostalom gdje će napraviti PCR? Ne može se pregled uvjetovati testom. Takva preporuka nije došla iz državne ni lokalne institucije, od ministra, ravnatelja, Zavoda za javno zdravstvo niti mene kao šefa službe, jer je to apsurd. PCR test tražimo samo kad dijete legne u bolnicu i za djecu stariju od sedam godina koja dolaze na spirometriju. Dakle, PCR se ne traži za pregled nego dijagnostiku u ovom slučaju, jer imamo samo jedan spirometar u kojeg pušu sva djeca i stoga se želi ukloniti rizik. To su moji kroničari s kojima imam dobru komunikaciju i oni to znaju unaprijed – rekla je prim. dr. Radonić.
